拥有无限光明前途的城南大学的研究生本乡猛（黄川田将也 饰），某晚在回家途中遭遇自称“修卡”的神秘组织成员。在被对方洗脑和改造后，本乡化身成为冷酷的假面骑士，替修卡干起各种罪恶勾当。曾采访过本乡的杂志记者绿川（小岭丽奈 饰）及其未婚夫矢野克彦（高野八诚 饰）在归家途中被可怕的怪人袭击。作为袭击者之一的本乡突然觉醒，背叛了修卡，但他也被绿川当作杀害克彦的凶手。内心受到巨大冲击的本乡，决定勇敢地站出来守护和平。未过多久，另一个假面骑士出现在他的面前，对方竟和克彦有着相同的容貌。修卡的邪恶计划仍在继续……本片根据石之森章太郎的经典漫画原作改编。
